Vibrios cholerae
- a disease of the poor and the third world, especially with indigenous populations
- need high infectous count/dose for disease
- bacteria usually hate acid, and survive better if stomach is alkaline
- antagonists which decrease stomach acid increases susceptibility to all enteric pathogens
- food has a buffering effect making the stomach vulnerable to bacteria
- death is caused by dehydration
- Cholera beds have a hole for your buns with a bucket underneath
- The goal is to allow bowel movements with "fire-hose diarrhoea" (not really stool; looks more like clear, foamy malodourous water)
- enterotoxin activated adenylate cyclase system
- cannot hydrolyse GTP → GDP
- constantly activated
- produces too much cAMP and causes hypersecretion of water and electrolytes, which results in secretory diarrhoea (important)
Vibrios parahaemolyticus
- Salt-loving
- commonly present in raw and undercooked shellfish.
- Common cause of GI problem in Japan due to the large consumption of sashimi (raw fish)
- Releases enterotoxin
- Also, raw oysters in New Orleans cause Vibrios parahaemolyticus infections.
Vibrios vulnificus
- aka monster of the deep
- Salt-loving, so salt as a disinfectant is not useful
- invasive, aggressive, causes fatal generalized sepsis
- Need immediate IV antibiotics
- Associated with sea water exposure (raw seafood/swimming in the sea)
- if a necrotic ulcer occurs after a history of exposure to sea water, give IV antibiotics immediately
- Vibrios vulnificus is killed by chlorine, so it is not a problem in public swimming pools
